The search strategies are displayed as entered into each database. Therefore, each line can be copied into the appropriate database to replicate the results if required. The search was constructed and tested in the OvidSP Medline database. This search was then edited to take account in differences of syntax and controlled vocabulary before being run in subsequent databases. The search was run in the following databases: OvidSP Medline; OvidSP Embase, OvidSP Global Health; OvidSP Social Policy & Practice; OvidSP PsycINFO; OvidSP Health Management Information Consortium (HMIC); Ebsco CINAHL Plus; Cochrane Library; Web of Science Core Collection; Scopus. In all searches, numbers in brackets after each line are the number of items retrieved. Results from each search are deduplicated against all previous searches. This is done by EndNote and manually, therefore, there may be some duplicates remaining in the EndNote file.
