The questionnaire provided here has been developed for use in workplace surveys in the United Kingdom. It is currently being trialled, and will be regularly revised and updated. It has been prepared as part of the work of the Partnership for Research in Occupational, Transport, Environmental COVID Transmission (PROTECT) initiative, which is part of the National Covid-19 Core Studies. The questionnaire is about exposures occurring during the past 14 days in current jobs. Questions may be modified depending on the nature of the study and the time period of interest. The focus is entirely on workplace and commuting recent exposures that may affect the risk of contracting COVID-19.
